IWTW’s Case Management Services
For those that need a helping hand beyond traditional, weekly therapy:
Invisible Wounds Therapy and Wellness can offer valuable, non-clinical assistance to clients, fostering independence and enhancing overall well-being.
Individuals requiring additional support beyond traditional therapy, such as those living alone, managing chronic mental health conditions, or needing assistance with activities of daily living or case management and navigating complicated systems such as the Veterans Affairs healthcare system, Medicaid and Disability may benefit from this service.
If you are not sure, please don’t hesitate to ask your therapist to be referred or simply reach out to the main office.
Service Components

Regular check-ins
Reinforcement of coping strategies or practicing skills in “real- time” with your case manager
Companionship, virtually or in- person when available, for those who struggle with severe depression, social withdrawal or loneliness

Traditional Case Management such as:
Assistance with scheduling appointments, medication reminders, and coordination of care
Assistance with collaborating with other providers, connecting to resources

Guided grocery shopping or assistance with creating grocery lists
Help with budgeting and financial literacy skills
Coaching through self- care and household tasks

Virtual or in- person (when available) accompaniment to social events, walks, or other activities to reduce isolation
Assistance with job searches, resume building and interview prep.
Resource building for community involvement such as volunteer work etc.
Delivery and Cost

Virtual Service: This means you will connect with your case manager virtually, either via video and/or over the phone. If needed, your case manager can connect you to in- person services with other providers or IW providers.
In- Person: When available, we may be able to connect you with a local case manager who can meet with you in our office or in your home.
Hybrid: A mix of both

Each Session, no matter the service delivery, costs $65.
This service is not covered by insurance and you are responsible for this payment.
***** You may be able to use your FSA or HSA cards to cover this cost, you are responsible to find out the eligibility for this.
Meet Your Case Manager
Hello, my name is Jasmine Young, and I am your Case Manager/ Behavioral Health Technician with a deep passion for helping individuals bridge the gap from wounds to wellness. I am a proud graduate of Howard University, where I earned my Bachelor’s degree in Political Science, and I later matriculated to Boston College, where I earned my Master’s degree in Cybersecurity.
With over 15 years of public service, I have built a diverse professional background that spans law, education, and the government. My journey has allowed me to combine leadership, advocacy, and compassion in ways that empower both individuals and communities.
Through my work with Invisible Wounds, I seek to bring together my experiences in behavioral health, cybersecurity, and public service to provide a holistic approach to healing. My mission is to support those facing unseen challenges, foster resilience, and create pathways for growth and wellness.
